Benefits are for “line of Duty” only for members in good standing.
All claims are for paid members in good standing. All claim forms MUST be signed and authorized by the Chief of the department. All claims will be verified for current membership prior to submission.
All claims MUST be submitted “within 60 days of injury”
Beneficiary forms are to be kept on file at the members ESO in a secure file.
